Social Support and Advice
The practice are able to offer support and advice from a team of four Social Prescribers working for Acclaim Primary Care Network (PCN).
Their role is to listen, help, advise and support you to improve your health and wellbeing. They will listen to the challenges you are facing, they will help you figure out what you would like to change, they will advise of services, groups and organisations that are available to help you achieve these changes and assist you in accessing support.
They can offer you either a telephone or face to face appointment at any of the PCN practices to discuss how they can help. This can be arranged by speaking to a member of your GP practice team who will then refer to the social prescriber team.
