Register with our practice

If you live within our practice area (Hull), you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill.  We also request one form of photo ID.

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Catchment Area (Practice Boundary)

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

You may be aware that from April 2015 all practices are required to provide all their patients with a named GP, who will have overall responsibility for the care and support that our surgery provides to them.

Your named GP will have overall responsibility for the care and support that our surgery provides to you. This does not prevent you from seeing any GP or Nurse in the practice as you currently do.

Accessing someone else's information

Accessing Someone Else’s Information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access:

  • Fill in our admin request form
  • Collect a proxy access form from reception from 10am to 6pm

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App
